From 2005 to June 30,
2010, management fee was charged at the annual rate of 2%. Beginning
July 2010, the managed account fee is 2.5% annual rate. The above
returns are actual account returns after fees and are not restated for the
management fee change. To adjust the
returns prior to July 1, 2010, to reflect the increased managed account
fee, reduce the monthly returns by 0.04% (four one hundreds of one percent,)
and reduce the annual returns shown by .5% (one half of one
percent.)
The Managed Account Returns in the
above table are based on actual accounts held in US Dollars. The managed account returns include dividends and
interest earned on accounts and are compared to the total returns of
the S&P 500 index (includes dividends) from published data. Income
taxes of the account holder are not reflected in the above table. All
accounts have their investments managed uniformly. The returns of
individual accounts that have deposits or withdrawals during the
period may vary due to the timing of the deposit or withdrawal.
The accounts invest in long
funds or short funds for U.S. equity indices. Accounts include leveraged
instruments (maximal leverage is two) such as mutual funds (e.g.,
S&P 500 2x STRATEGY RYDEX fund) and Exchange Traded Funds. All
accounts have the same underlying active trading strategy in US equity
indices.

Dear Investor:
Thank you for considering my
managed account plan. On this page is a brief explanation of my managed account
plan and my current table of managed account results.
Since August 1, 2005, I have been a
Registered Investment Advisor managing client speculative funds using
Stock Index ETFs and Mutual Funds. Client accounts are held at a Broker Dealer. Currently I am registered with
Rydex Funds (for US accounts) and Interactive Brokers (for non-US
accounts) accounts. The account is "your"
individual account or "your" IRA. I take a "trading power" over the
account that limits me to only placing orders on the account and to submit my
management fee. The market timing trading strategy followed in the accounts
includes using leveraged long or short funds and ETF positions, but does not necessarily
exactly match the recommendations in the weekly Stock Index Timing .com
newsletter.
When you set up an account at
Rydex or Interactive Brokers and give me trading power of attorney, the account gets set up in my
managed "group" and this lets me move between long or short
the market for all the accounts with just a few keystrokes online. At
Rydex, my
2.5% annual fee is pro rated monthly and charged at the end of each month. Interactive Brokers pro rates my
2.5% annual fee to a daily charge.
There are no transactions
fees at Rydex. Transaction fees at Interactive
Brokers are the lowest rates I can find. With very low or no
commissions, I can go back and forth
without hesitating about transaction costs being too high for small
accounts.
You can see that my returns are very speculative.
The goal of my managed account plan is to profit in almost any market
circumstance. I
suggest you only let me manage funds like they were just one
speculative investment in one stock. For example, if you have a million dollars in
stock market investments, I suggest you put $50,000 to $100,000 in
this program.

August 2005 to July 2006 were at Brown Co., E Trade, and Scott
Trade and are not available on line for independent access and so Theta only
reviewed the Rydex statements.)

Note: The Theta Research results
have four individual months that have returns that vary from the above
(one by less than one percent, and three by less than one half
of one percent) because they assume deposits at the end
of the month and withdrawals at the beginning. This skews
performance slightly depending on the size of the cash flow.
This is acceptable because they are not trying to provide
audited records. They are trying to give prospective advisors
and clients a good look at how the model trades in real time.
These slight variations are not critical to that purpose.
Theta's S&P 500 comparison reflects the index price changes,
and my comparisons above reflect total return changes (price and
dividends.) |

1. Review the Investment Advisor Form ADV Part
II Disclosure.
Before opening an account, please click here to
review the Investment Advisor Form ADV Part II Disclosure
(3 meg file).
2. Complete the linked client Agreement.
The Please print out the linked six page file.
First, is the three page Investment Advisor
Agreement - please review, then sign and date.
Next is the one page New Client Questionnaire
- please complete and sign and date.
Then, the two page Rydex Appointment of
Financial Professional form - please fill in the account name and sign and
date.
After completing these, please
mail, fax, or email these to George Slezak at the address on the pages.
3. Open an account at Rydex SGI
Funds (or for international accounts at Interactive Brokers.)

The following information relates to opening
accounts at Rydex SGI Funds
Currently,
my investment plan includes moving between fund accounts either in the morning
or at the close of the day. I only know of the Rydex SGI Mutual Fund Group offering
the ability to trade mutual funds at both a morning close, at 10:30 am Eastern,
and at the daily close. Accordingly, I currently suggest clients open accounts
at Rydex Funds to participate in my managed program.

The following information relates to opening
accounts at Interactive Brokers
Interactive Brokers has a complete on line
account opening program.
To initiate the account opening, George
Slezak needs to send you an email through Interactive Brokers to invite you to
join his managed account group.
Send George an email requesting an Interactive
Brokers account invitation. Send
to george@georgeslezak.com Include
the name of the person opening the account (or owner of the IRA) and the email
for the invitation, and the size of account you intend to open. If you are
making an IRA transfer to open the account, the instructions for making an IRA
transfer is part of the online forms.
