PRINTING You need to ONLY PRINT pages 3 to 6 and pages 9 and 10 of this account booklet. Also print the following two pages (2 pages) Appointment of Financial Professional Filling in the forms: Page 3, box 1 Check Traditional IRA. Fill out info. Page 3, Box 2 Check transfer and fill in amount. Page 4, Box 3 Leave blank Page 4, Box 4, Beneficiary Information Complete information Page 4 and 5, Box 5, Account Options and Services Leave entire box blank Page 6, Box 6 Leave Blank Page 6, Box 7 Signature and date Page 7 and 8 are pages that both of us need to complete. I have partially completed these two pages. Print out the following linked form (2 pages) Appointment of Financial Professional. This form appoints me as your agent for purposes of giving Rydex instructions for transactions in your account. It also allows the payment of my 2% per year management fee which is charged as .5% of the calendar quarter end balance just after the end of each calendar quarter. Sign page one and send both pages to Rydex.
Page 9 is the IRA transfer form. Complete part 1 Complete part 2 Complete part 3 Check partial transfer, indicate amount, and indicate to liquidate from money market or other fund. Check immediately Check "from" appropriate fund. Check "to" appropriate fund.
Page 10. Complete part 4 check "by wire. " Complete part 5 Sign and date. If the "medallion" signature guarantee is needed, Rydex will contact us after they receive the forms. Attach a copy of the existing IRA statement.
