PRINTING You need to ONLY PRINT the first FOUR pages. (Note: The 2 page Appointment of Financial Professional was completed with our agreement.) Sign on the last pages and mail, with your check, to the address on the form. Call if you have any questions! George 1-888-311-3400

Page 2. Just one check on the entire page.
Page 3. Just three checks at the top pf page 3.
Page 4. Just signatures and date at the bottom of the page.
